Follow the instruction on How to Order Gift(s) in this Gift Order Form (PDF) Visit our Project Catalog and read about the projects you can support Choose the project(s) you would like to gift in honor of someone Fill out the Gift Order Form and send to MEDRIX with payment Your gift card will then be mailed directly to you or to the honoree as you choose on the Gift Order Form
